What a clinic consultation and consent form needs
Split the form into sections a client can follow: contact details, medical history and current medicines, allergies, previous treatments, treatment goals, then consent statements and a signature. Mark the answers your practitioner can't do without as required, so every client answers them. Add a practitioner section for notes and their own signature.
Start from a treatment template
Write the consent wording your business has approved, or start from a template for anti-wrinkle, dermal filler, microneedling, PMU or lash treatments and edit it with your practitioner.
How forms fit the clinic day
A client books a dermal filler appointment for Thursday. Reception sends the consultation form by link or SMS that afternoon, and the client completes it on her phone that evening. On Thursday the diary shows the form as completed next to her deposit, and the practitioner reads her answers and previous notes before calling her in. Attach the form to a service and clients receive it with their booking confirmation.
Walk-ins and tablets
Walk-ins and clients without a phone can fill in the form in person on a tablet at reception. Either way, the signed form stays on the client record for the next visit.

Move from paper or another forms system
Start by moving the forms you use most, then retire the paper versions one treatment at a time. If your signed forms sit in another system, data migration is free on every plan, including photos and attachments, and setup help is free, with next-day setup possible. We'll handle the import for you.
Consent record, not a replacement for consultation
A signed form gives your practitioner the client's answers and a record of consent. It doesn't replace the consultation. Your practitioner still assesses the client and decides whether treatment goes ahead. On Growth, clinical governance shows any outstanding forms for a service before treatment starts.
